Clinic Support (Staff) Pharmacist
Clinical Services Handles routine medication-related activities. Makes recommendations to clinical staff regarding medication therapy. Documents recommendations and interventions. Provides refill extensions and partials as appropriate. Assess and documents Adverse Drug Events. Performs medication reconciliation. Assists in formulary management to include therapeutic substitutions. Provides oversight of technical staff in all aspects of medication distribution. Medication Dispensing and Distribution Accurately fills or checks medication dispensing units, prescriptions, and aseptically prepared intravenous admixtures and other aseptically prepared pharmaceuticals. Fills or causes to be filled all physician orders and requests (inpatient and outpatient) in a timely manner. Utilizes VistA and CPRS for order entry and completion. Takes and passes an annual aseptic technique and chemotherapy preparation test, to certify knowledge of aseptic technique. Administration Documents all patient medication teaching sessions and consults in the patient electronic record. Keeps necessary records and reports in an accurate, timely and complete manner. Provides strict security of controlled substances by reviewing all orders, issues and receipts to prevent alterations, forgeries and pilferage. Has a thorough knowledge of the rules and regulations regarding dispensing of controlled substances. Provides quality control analysis for dispensing activities of automated dispensing equipment and pharmacy technicians. Education and Training Provides drug information and education to medical staff, nursing staff, pharmacy staff, other health care personnel, patients and their representatives on medication-related topics as needed or assigned. This may include both informal or formal presentations. Assumes an active role in the overall pharmacy-training program by providing orientation, guidance and training to pharmacists, technicians, students, residents, etc., to ensure that pharmacy and medical center procedures and techniques are shared and learned to the necessary degree. Acts as a preceptor or mentor for pharmacy student and resident rotations. Participates in staff development of peers and collaborates with other pharmacists in the provision of specialized training programs and assists in the development of innovative training concepts aimed at improving the knowledge and quality of pharmacy and other clinical staff. Maintains basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in the area of practice. Sustains professional competency by maintaining awareness and knowledge of the latest developments in pharmacy practice and drug therapy by reading pharmacy and medical literature, and participating in continuing education programs. (1) Graduate of a degree program in pharmacy from an approved college or university. (2) Graduates of foreign pharmacy degree programs or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. see below. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service. GRADE DETERMINATIONS:
CREDITABLE EXPERIENCE. 1) Knowledge of Current Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. (2) Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). (3) Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.
