Role Description This is a full-time on-site role for a Home Health Licensed Practical Nurse. The Licensed Practical Nurse will be responsible for providing skilled nursing care to patients in their homes. This includes administering medications, monitoring patients' vital signs, dressing wounds, assisting with personal care, and educating patients on self-care techniques. The Licensed Practical Nurse will work closely with other healthcare professionals to develop and implement individualized care plans for each patient. Qualifications Licensed Practical Nurse (LPN) with a current and active license Experience in nursing and patient care Knowledge of medical practices and procedures Experience in home care or a related field Strong communication and interpersonal skills Ability to work independently and as part of a team Excellent organizational and time management skills Ability to provide compassionate and empathetic care Experience with electronic medical records is a plus Recommended Skills Communication Home Care Interpersonal Skills Licensed Practical Nurse Medical Practices Nursing Apply to this job.
