What you will do:
Provide support to patients and families Assist patients and families in obtaining needed services Conduct initial psychosocial assessments of new patients Travel to patients wherever they reside:
Home, nursing facility etc.
Requirements:
What you need to have:
Master of Social Work (MSW) Valid social work license Driver's license, insurance and vehicle 1-2 years of healthcare experience (preferred) Utmost compassion and integrity
